Additional Billing Info & Fees

  • Your copay or portion of your deductible is due at the time of service unless arrangements have been made prior to your appointment.
  • If your account balance is over $150 and/or no payments have been made for more than 60 days, the provider may not continue to provide services until a payment has been made or arranged.
  • There is a $30.00 fee for checks written with insufficient funds.
  • Balances past due for 90 days may be sent to an outside collections’ agency.
  • There is a $25 fee for FMLA forms to be filled out and will be done at the discretion of the provider.

Bluegrass Behavioral Health & Consulting Services accepts cash, check, or money orders as well as:

A valid credit/debit card is required to be on file when setting up an appointment for self-pay clients and will be charged in the event of a no show for the full amount of the visit which is non-refundable ($250 for new client visits and $150 for follow up appointments).
